3/30/2023 EnchantaVaughn Approved Behavior: Client shared that she was ready for the session. Client appeared to be in good spirit and happy as evidenced by smiling and being eager to start the session with counselor. Client’s mother appeared calm and engaged in full conversation with counselor about her day and the current struggles of managing emotions she was feeling. Behavior: Client have been proactive about working on herself and presenting a better attitude in the home. Client was continuing to work on being able to learn how to communicate with her mother and other as she struggles with this skill. Intervention: QMHP-C reviewed key concepts of being responsible with her family. QMHP-C educated Client about being respectful, encouraging, mindful, and fair to peers and authority while making good decisions in the home. QMHP-C will provide mediation and relaxation techniques, as well as, simple praises and encouragement to continue the positive behaviors she was exhibiting. QMHP-C aided this intervention with Client on social skills. QMHP-C introduced Client to psycho social activity regarding hygiene and staying healthy. QMHP-C educated Client that it is vital to wash your hands and stay inside. QMHP-C probed Client about her ability to not get involved with a negative crowd. QMHP-C discussed with Client the importance of conducting herself appropriately at all times. QMHP-C utilize positive self-talk strategies to help Client think positively at all times and to talk about her actions and how they will make people feel. Intervention: QMHP-C utilized active listening skills while Client was verbalizing information about her day. QMHP-C explored Client’s capability to cope with her stressors and to get back on track. QMHP-C assisted Client with finding and utilizing necessary resources at home. QMHP-C advised Client to make the right choices and to keep up the good work. QMHP-C probed Client about her schedule in efforts to understand her daily routine. QMHP-C probed Client to not be afraid and ask questions if she doesn’t understand any of the in-home assignments so that she will not have the same failures once she goes back to normal activity. Response: Client was assisted in practicing general communications skills. Client understood effective problem-solving skills to help solve or overcome a problem or stressor that she is currently facing. Client understood that when she becomes agitated she needs to refrain from taking it out on people and to communicate how she is feeling to others. Client communicated positive affirmations and self-talk once she was able to let go of negative, agitated feelings. Client shared some of the things she does at home to help her relax. Client participated in reviewing key concepts. Client showed good understanding of what it means to be respectful, encouraging, mindful, and fair to others. Client is observed correcting her mistakes and making good choices. Client was able to utilize coping skills to eliminate stressors and unwanted feelings. Client utilized meditation and relaxation techniques.
